All patients must complete our Patient Registration form before seeing the doctor.

Financial Policy

Payment in full is required at the time of service. Refusal to pay for services rendered or to make a financial arrangement will result in an account being sent to an outside collection agency. This may harm an individual’s credit status.

Minors

Written consent from a legal parent or guardian is required if they do not accompany the minor. The adult accompanying the minor is responsible for full payment at the time of service.

Appointments

We may call in advance to confirm your appointment. This is only a courtesy, as the responsibility remains with the patient to manage his/her appointments. If you need to cancel an appointment, please give 24 hours’ notice.

Forms Filled by Doctor

Work or insurance-related forms (such as FMLA) may be filled by the doctor for a $25.00 fee paid by the patient in advance. Please allow sufficient time for this request.

Transfer of Records.

If you request the transfer of records to another doctor or medical facility, there will be no charge. Please fill and sign the form – “Release of Medical Records.” Please allow time for this request.

Not every Annual Wellness Visit is the same. As part of our effort to build great doctor-patient relationships, we want to take a moment to prepare you for your wellness visit and the possibility of a co-pay. Most importantly, we want you to understand that there may be out-of-pocket-costs associated with your annual wellness visit.

ANNUAL WELLNESS VISIT

An Annual Wellness Visit is meant to be an opportunity for the doctor to get an overview of the patient’s health status, including elements of physical and mental health and general safety. This includes a review of the patient’s known chronic conditions.

Annual wellness visit will also include a standard office visit, preventative care, and diagnostic care. According to your insurance, these medically necessary or diagnostic services are considered added services that are not part of a standard annual wellness visit.

What is the difference between Preventative Services and Diagnostic Services?

  • Preventative Services – Preventative care is given to a patient when they are symptom-free and have no reason to believe they might be unhealthy. This type of preventative care is given as part of an annual wellness visit.
  • Diagnostic Services – Diagnostic care is what you receive when you have symptoms or risk factors. Your provider needs to diagnose them so they can accurately identify and treat your condition.

The primary reasons to administer the additional services at the time of your annual wellness visit are three-fold:

  • To address a new or existing problem before it becomes aggravated.
  • To eliminate the need for an additional office visit
  • To continue to improve each patient’s long-term health, wellness, and happiness.

  • Ordering and Reviewing Diagnostic Tests
  • Renewing Prescriptions
  • Making Referrals
  • Implementing Other Changes to Your Treatment or Care Plan
  • Administering Vaccines
  • About Your Annual Wellness Visit

This preventative medical evaluation typically includes:

  • A review of the patient’s medical history: surgeries, illnesses, hospital stays, allergies, injuries, treatments, and immunization records.
  • A review of a patient’s existing health problems
  • A review of the patient’s family medical history
  • A review of the patient’s Medications and Supplements
  • Checking routine measurements; height, weight, body mass index (BMI), and blood pressure
  • Collecting information about current treatments
  • Identifying any risks
  • Suggesting a preventative care plan – Preventative care is the cornerstone of family medicine.

Who is eligible for the Annual Wellness Visit?

Most insurance companies provide this benefit. This is a proactive preventative measure meant to detect any issues before they become difficult to treat.
